Is the information that Adult Care and Family Care Homes collect and share with their Quality Improvement Consultant (QIC) kept confidential?
Yes. The QIC must keep confidential the materials that are produced and evaluated during the program (General Statute 131D-21.2).
There are exceptions only for the QIC's responsibility to report abuse, neglect, or exploitation as required by General Statute 108A, Article 6. In addition, a consultant must immediately report to the Adult Homes Specialist any incidents of death, risk of death, and serious physical harm uncovered during on-site provision of quality improvement activities.
Who are the Quality Improvement Consultants (QICs)?
QICs are experienced Adult Homes Specialists working in their county's Department of Social Services. They have received training in the quality improvement methods used in this program.
It is important to note that a QIC does not provide consulting services to a home which he or she monitors as an Adult Homes Specialist. The work that a QIC does with a home participating in the Adult Care Home Quality Improvement Consultation Program is kept confidential.
What do I do with Action Plans once they are developed with the home?

On the Medication Management Self-Assessment, a home I’m working with wanted to answer “not applicable” to some of the questions, but that option isn’t available. What do I do?
If a home needs to answer “not applicable” to more items than the assessment currently allows (currently, a home can say only that sections “3. Telephone Orders” and “6. Pre-poured Medications” are not applicable to them). Follow these instructions:
- If a given question **truly** does not apply to the home because of legitimate policies or characteristics of the residents they care for, have them answer “Yes” to that question. This ensures that their score for that category isn’t “dinged” for something that truly doesn’t apply in their home.
- To help us learn from your experience and improve this tool, email both Emily Saunders at the Division of Aging and CCME to let us know what those questions were. Emily may need to get more information from you, and we at CCME need to keep log of this information.
