The Carolinas Center for Medical Excellence (CCME) is committed to maintaining the trust and confidence of
visitors to our Adult Care Home Quality Improvement (ACHQI) website. We have created this web policy to
demonstrate our firm commitment to your privacy and the protection of
your information.
Personal Information
You do not have to give personal information to visit the public portion of the ACHQI website.
This public section gives visitors the chance to read and obtain more information on the ACHQI
program and CCME. Additional activities on our site require you to be a registered participant
of the Adult Care Home Quality Improvement Consultative Program.
Participants in the Adult Care Home Quality Improvement Consultative Program must register in
order to access the data collection and/or reporting section of this website. As part of the
registration process, you will be asked to provide information about yourself and your organization.
This information is collected to make sure that only authorized persons have access to data and
reports about your organization. Once your registration is approved, you will be assigned a unique
user ID and password,which will allow you to log in and control the data and reports to which
you have access.
Non-Personal Information
We analyze traffic to this website in order to understand our visitors’ and program participants’
needs and to continually improve our site for them. We do not share it with third parties.
The ACHQI website automatically collects and records non-personally identifiable information
that becomes available to us as a result of your visit to our site. This information includes:
date and time of visit, pages visited on our site, type of computer and browser the visitor is using,
and the location of visitor’s machine (by country, not street address).
Many websites gather this information, though most do not notify visitors.
ACHQI Cookies
A cookie is a small piece of data that is sent to your Internet browser from a web server and stored
on your computer’s hard drive. A cookie cannot read data off of your computer hard disk or
read cookie files created by other websites. Cookies do not damage your system.
